> All documents must be originals except for training certificates and CPR/First Aid card > An original and recently generated State of Alaska Background Check is required. > Must be supervised by CDCS or higher. > Practicum letter must be written on company letterhead documenting the completion of the specific criteria requirements listed on Matrix.
